Note (* All field are mandatory.)

Please attach following documents along with the Admission Form :

1. Self attested photo copy of the birth certificate, Please also produce original birth certificate for verification.
2. In case if the student has attended any other school before, please submit self attested photo copies of:
   School leaving certificate
   Migration certificate, if applicable
   The last term / annual result.
3. Coloured passport size photographs - 4 nos.
4. Self attested photo copy of Adharcard / Passport / Electric bill / Driving licence as Permanent residential address proof.

INSTRUCTIONS

1. Please fill up the form in legible and capital handwriting.
2. Incomplete form is liable to be rejected.
3. Admission fee & first quarter fee is to be deposited with the admission form, No admission is final till the fee is paid.
4. Admission fee is non-refundable under any circumstances once admission is confirmed and fee is paid.
5. Any kind of indecent behavior by parents /student in the school premises can possibly reject the admission in future.
6. Admission confirmation is subject to submission of all specified necessary documents within stipulated time frame.

MAJOR SCHOOL RULES :

1. In case of change in temporary / permanent address or phone /mobile number, the school should be informed immediately in written.
2. It is compulsory to attend all meetings organized for parents / guardian.
3. There will be raise of minimum 7.5% in the fee every year.
4. Appropriate school leaving certificate or any other certificates will not be issued unless:
   - Admission withdrawal request is submitted in written and signed by the parent's.
   - All the dues are paid.
5. The school management will not be responsible for any loss or injury to the student during the school
   hours or course of any activities conducted by the school.
6. Admission confirmation is subject to submission of all specified necessary documents within stipulated time frame.

DISMISSAL :

1. Irregular attendance, habitual idleness, disobedience, indecent behavior, lack of sufficient improvement in
   studies, any one of above is sufficient reason for dismissal of admission of the student.

Fee Submission Dates:

1. 1st Quarter Fee : 15th May to 31st Jun of current academic Session.
2. 2nd Quarter Fee : 15th Aug to 30th Sep of current academic Session.
3. 3rd Quarter Fee : 15th November to 30th December of current academic Session.
4. 4th Quarter Fee : 15th Feb to 31st March of current academic Session.
